Coronavirus Self-Declaration Form

Due to the ongoing and rapidly changing situation with the novel – coronavirus (COVID-19), we are
requiring all visitors to Cod Beck Blenders premises to fill out the self-declaration form downloadable below. Cod Beck Blenders will then decide on access to the premises based on the answers provided in the declaration.

Each visitor must complete this form prior to your appointment, meeting or visit please email the form to blongthorne@cbchem.co.uk and also the contact person in Cod Beck Blenders that you have an appointment, meeting or visit with.
